I, for good and valuable consideration, hereby grant permission to Audio Information Network Of Colorado. Inc dba Aftersight (“Aftersight”) to the irrevocable right to use photographs, portraits, pictures, images, likenesses, audio and/or video recordings of me (herein collectively referred to as “image and likeness”) on Aftersight and other websites and in publications, promotional flyers, materials, derivative works, or for any other similar purpose without compensation to me.

I acknowledge, understand, and agree that my image and likeness may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my image and likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image and likeness. I also understand that this material may be used in a multitude of diverse settings within an unrestricted geographic area.

I acknowledge, understand, and agree that I will not be consulted about the use of the photographs or video recording, and my image and likeness, for any purpose.

I acknowledge, understand, and agree that this permission signifies that my image and likeness may be electronically displayed via the Internet, in other forms of print, media, digitally, or in other public and private settings.

I also acknowledge, understand, and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany my image and likeness. I waive the right to approve the final product. I agree that all such images and likeness, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of Aftersight.

I further acknowledge, understand, and agree that there is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be used and distributed.

I hereby release, acquit and forever discharge Aftersight, its current and former directors, officers, employees, agents, assigns, participants, volunteers, donors, and fundraisers from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said image and likeness, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.
